2008 Healthy Teen Network Annual Awards Program
Call for Nominations

 
Outstanding Teen Parent

This award is presented to a former teen parent who has overcome the obstacles of very early parenting to achieve extraordinary personal and/or professional success. Typically, this person is at least 30 years old, self-sufficient, and demonstrates outstanding leadership in achieving community or career goals. This person may or may not be a Healthy Teen Network member.

Outstanding Emerging Innovation
This award is presented to a program in existence for less than three years. The award winning program will be comprehensive in scope, innovative, linked with other appropriate public or private agencies and could possibly be replicable in other areas.

Eligibility Criteria
1. A demonstrated commitment to the prevention of teen pregnancy or excellence in teen pregnancy and parenting services is necessary. We are looking for nominees who have made outstanding contributions toward these goals.
2. Evidence of leadership, creativity, organizational involvement and effectiveness.
3. Award recipients must be current members of Healthy Teen Network.
4. All recipients must make a commitment to attend the Healthy Teen Network Annual Awards Ceremony at their own expense.
5. Programs and Coalitions employing Healthy Teen Network Board Members are not eligible to apply for awards.

Nomination Procedure
To make a nomination for any of the awards, follow the steps below. Self-nominations are accepted.
 
All materials must be received by August 15, 2008.
 
1. Complete the 2008 Healthy Teen Network Award Nomination Form. (Click here to download form.)
2. Submit a Letter of Nomination not exceeding two typewritten pages in which the following are specifically addressed:
* a brief history of the nominee's involvement in the field and summary of their focus within the field;
* the scope of the organization's or individual's work in the field;
* clearly stated reasons for the nominee's selection.
3. Submit three letters of support for the nomination, two of which must come from outside the nominee's organization.
4. Optional: Attach any other appropriate materials (e.g., news articles, brochures, etc.) that support the nomination.

Selection Process
* Selection Committee (professionals with expertise in relevant fields) reviews applications and nominates award recipients.
* Final selection of award recipients is made by the staff of Healthy Teen Network.
* Award winners are notified no less than thirty days prior to the Awards Ceremony.
